  • Participate in interdisciplinary and virtual ward rounds, managing complex cases across multiple specialties.
  • Optimise and individualise medication regimens to improve safety, adherence and clinical outcomes.
  • Provide expert consultation on medication management and evidence-based practice.
  • Educate patients and caregivers to support effective therapy management at home.
  • Contribute to clinical governance, protocol development and service innovation.
  • Maintain ongoing professional development to remain at the forefront of healthcare innovation.
  • Deliver comprehensive clinical pharmacy services, including medication reconciliation, therapy monitoring and documentation.
  • Identify and resolve drug-related problems, ensuring compliance with prescribing guidelines and protocols.
  • Provide evidence-based drug information and collaborate closely with multidisciplinary teams, patients and caregivers.
  • Lead or contribute to education programmes, professional standards, competency frameworks and continuing development initiatives.
  • Initiate or participate in clinical research and contribute to publications or scientific presentations.
Requirements
  • MSc (Clinical Pharmacy), PharmD or Board Certification.
  • Collaborative Prescribing Course completed within the last 2 years.
  • Minimum 5 years of clinical pharmacy experience (preferably inpatient).
  • Registered with the Singapore Pharmacy Council.
  • Strong leadership, clinical judgement and evidence-based practice skills.
  • Excellent communication, supervisory and stakeholder management abilities; additional language skills are an advantage for liaising with diverse clients
  • Passion for transforming healthcare through innovative care delivery.
